As a client participating in the Virtual Alcohol and Recovery Program, it is essential to understand the nature of the services provided and the terms and conditions under which these services are offered. By signing this agreement, you acknowledge and agree to the following items:
The program employs various virtual treatment strategies that may be helpful in addressing chemical dependence issues; however, specific outcomes cannot be guaranteed.
Adherence to certain ground rules is crucial for a successful virtual treatment experience. Violating these rules may lead to termination of treatment.
I, the client, agree to the following:
a. I will log in on time for virtual appointments and be prepared to submit virtual assessments and self-reports as required.
b. I commit to abstaining from all drug and alcohol use throughout the entire virtual treatment program. If I cannot make this commitment, I will discuss alternative treatment options with the program staff.
c. I will discuss any drug or alcohol use with the staff while in virtual treatment.
d. Treatment consists of individual virtual sessions. Virtual appointments can be rescheduled if necessary, but I understand that attendance is vital. I will notify the counselor in advance if I am going to miss a virtual session.
e. I understand that attempting to promote drug use or engage in inappropriate behavior during virtual sessions will result in treatment termination.
f. Graphic stories of drug or alcohol use are not permitted during virtual sessions.
g. I acknowledge that all matters discussed in virtual sessions are strictly confidential and will not share this information with others.
h. I recognize that treatment is voluntary, and if I decide to terminate treatment, I will discuss this decision with the staff.
Services are provided by qualified psychologists, licensed marriage and family counselors, master’s-level counselors-in-training, or other certified addiction staff. Non-licensed counselors are supervised by a licensed counselor trained in addiction treatment.
To ensure high-quality services, virtual therapy sessions may be recorded for training purposes. Clients consent to the recording of sessions.
All information disclosed in virtual sessions is strictly confidential and may not be revealed to anyone outside the program staff without written permission, except when disclosures are required or permitted by law, typically involving substantial risk of physical harm or suspected abuse.
Achieving treatment goals necessitates the cooperation and active participation of clients. In rare cases, lack of cooperation may hinder the program’s ability to provide services effectively, potentially resulting in discontinued services.
